A disciplinary panel later found that the pediatric registrar failed to recognize and record key symptoms and did not properly assess the child's condition or arrange further tests. Harry's condition worsened at home, and he was found stiff and unresponsive early one morning, with resuscitation efforts proving futile. The hospital admitted that a series of procedural failures occurred and later issued an apology to the family, also implementing systemic changes to improve pediatric care as a result of this tragedy.
