To apply for an Ayushman Bharat card, follow these steps:
- Check your eligibility first. Eligible families are usually listed in the Socio-Economic Caste Census (SECC) 2011 database. You can verify online on the official PMJAY website, by calling the helpline 14555, or visiting your nearest Common Service Center (CSC).
- Gather the required documents including Aadhaar card of family members, ration card if available, mobile number linked with Aadhaar, caste certificate if applicable, income certificate if required, and bank account details.
- Apply either online or offline:
- Online: Visit the official PMJAY website, fill the application form with necessary details, complete e-KYC via Aadhaar OTP, and submit the form.
- Offline: Visit the nearest CSC, empaneled hospital, or Ayushman Mitra with the documents. The staff will assist you in filing the application.
- After submission, your details are verified against the SECC database, usually within 15-30 days.
- Once approved, you receive the Ayushman Bharat Golden Card digitally on your mobile or physically by post. Cards can be collected at CSCs as well.
- The card covers the whole family listed in SECC data and can be used at any empanelled hospital in India for cashless treatment.
No fees are charged for the application process.
