1. Prepare the Required Documents:
- Fully accomplished UMID Card Application Form (can be downloaded from the SSS website).
- Original and photocopy of one primary valid ID such as:
- Passport
- Driver’s License
- SSS Card
- Postal ID
- Voter’s ID
- NBI Clearance
- Seaman’s Book
- If you do not have a primary ID, bring two secondary valid IDs with your photo and signature (e.g., school or company ID, senior citizen card, TIN ID, birth certificate)
2. Schedule an Appointment or Walk-in:
- You can schedule an appointment online via your My.SSS account for biometric capture at your preferred SSS branch.
- Alternatively, you can walk in at any SSS branch that accepts UMID applications, but be aware of the branch's number coding system for walk-ins based on your SSS number
3. Submit Your Application and Enroll Biometrics:
- Bring your completed application form and IDs to the SSS or GSIS branch.
- The staff will verify your documents and capture your photo, signature, and fingerprints.
- You will receive an acknowledgment receipt after biometrics enrollment
4. Wait for Notification and Claim Your UMID Card:
