To apply for a passport, you will need the following documents:

Proof of U.S. citizenship: You must submit an original or certified physical copy of a document that proves your U.S. citizenship. Acceptable documents include a U.S. birth certificate, Consular Report of Birth Abroad or Certification of Birth, Certificate of Naturalization, or Certificate of Citizenship.

Photo identification: You must present one of the following identification documents (IDs) to the acceptance agent. The ID must readily identify you. Acceptable IDs include a driver’s license, government-issued ID, military ID, Certificate of Naturalization, or previously issued passport (if you are applying for a new one) .

A photocopy of both your citizenship document and photo ID: You must submit a photocopy of the front (and back, if there is printed information) of each ID that you present when you apply. Photocopies must be legible, black and white, on plain white 8 1/2" x 11" standard paper stock, and taken within the past 6 months.

A qualifying photograph: You must provide one color passport photo that meets the State Departments requirements.

Passport fees: You will need to pay the appropriate passport fees, which vary depending on the type of passport you are applying for and how quickly you need it.

Additionally, if you cannot present one of the IDs above, or for more information, you should see Identification. If you are a minor or first-time applicant or your previous passport was issued more than 15 years ago, you must appear in person and complete a passport application (Form DS-11) using black ink.
