Jeffrey Epstein was found dead in his cell at the Metropolitan Correctional Center in New York on August 10, 2019. He was awaiting trial on federal sex trafficking charges at the time of his death. The official cause of death was suicide by hanging. Epstein had previously attempted suicide on July 23, 2019, and was placed on suicide watch. However, he was taken off suicide watch six days later and was not being monitored at the time of his death. A new Justice Department Inspector General report revealed that Epstein hanged himself in his jail cell with an orange noose he fashioned from "a sheet or a shirt". The report also detailed the failures that occurred beginning one month before Epstein died. Negligence, misconduct, and job performance failures all contributed to Epsteins suicide, according to the report. The report highlighted systemic problems at the Federal Bureau of Prisons as well as several issues at the Metropolitan Correctional Center. Two correctional officers were later charged for failing to check on Epstein the night before his death.
