"What If... Doctor Strange Lost His Heart Instead of His Hands?" is the fourth episode of the first season of the American animated television series What If...?, based on the Marvel Comics series of the same name. In this episode, Dr. Stephen Stranges girlfriend Christine Palmer dies instead of Strange losing the use of his hands. A grief-stricken Doctor Strange uses the Eye of Agamotto in a dangerous effort to change the past, leading him down a dark path to become Supreme. He discovers that the accident that killed Palmer is a fixed point in time, and he cannot change it. In his desperation, he begins to absorb the powers of other life-forms to break Absolute Points.

In the end, Strange becomes an alternate version of himself known as Doctor Strange Supreme, who is willing to destroy his own universe to save Palmer. The episode explores the consequences of Stranges actions and the dangers of meddling with time and the multiverse. It is suggested that this episode could tie into upcoming MCU movies Spider-Man: No Way Home and Doctor Strange in the Multiverse of Madness.
