The role of a service coordinator can vary depending on the industry they work in. For example, in the healthcare industry, a service coordinator is responsible for assessing the service needs of clients and linking them to the appropriate resources and providers. In the housing industry, a service coordinator manages and provides access to necessary supportive services in the community. In Texas, a service coordinator is assigned to each enrolling Home and Community-based Services (HCS) applicant and HCS participant to assist them in designating members of their service planning team (SPT) and accessing the necessary resources.

The responsibilities of a service coordinator can include:

• Assessing the service needs of clients and linking them to the appropriate resources and providers
