A wellness visit is a yearly appointment with a primary care provider (PCP) to create or update a personalized prevention plan based on your current health and risk factors. It is not a head-to-toe physical exam, but rather a health evaluation that may help prevent illness. The specific components of a wellness visit may vary depending on the provider and the patients age, medical history, and risk factors, but generally include the following:
- Review of medical and surgical history
- Screenings, such as blood tests and immunizations
- Physical exam, which may include measurements like height, weight, and blood pressure
- Counseling to prevent future health problems
During the first wellness visit, the PCP will develop a personalized prevention plan, which may include a health risk assessment, review of functional ability and level of safety, and screening for hearing impairments and risk of falling. The visit may also include a questionnaire that asks about health status, injury risks, behavioral risks, and urgent health needs. Medicare Part B covers the Annual Wellness Visit if certain eligibility criteria are met.
