CCM stands for Chronic Care Management, which is a collection of resources available to Medicare beneficiaries with two or more chronic conditions. CCM services are generally non-face-to-face services provided to Medicare beneficiaries who have multiple chronic conditions expected to last at least 12 months or until the death of the patient. The Centers for Medicare & Medicaid Services (CMS) recognizes that CCM services are critical components of primary care that promote better health and reduce overall healthcare costs. CCM codes can be billed for services furnished to patients with two or more chronic conditions who are at significant risk of death, acute exacerbation or decompensation, or functional decline. CCM services include personalized assistance from a dedicated healthcare professional, 24/7 emergency access to a healthcare professional, and coordination of care between the patients pharmacy, specialists, testing centers, and hospitals. The five CPT codes used to report CCM services are 99490, 99489, 99487, and HCPCS code G0506. Only one practitioner per patient is eligible to receive CCM payments over a given calendar month.
