SPH (Sphere): This measurement indicates the lens power required to correct nearsightedness or farsightedness. The number is measured in diopters, and a minus sign (-) indicates nearsightedness, while a plus sign (+) indicates farsightedness.

CYL (Cylinder): This measurement indicates the lens power required to correct astigmatism, which is an uneven curvature of the eyes cornea or lens. If this column is blank, it means you have no astigmatism. A minus sign (-) means your prescription corrects nearsighted astigmatism, while a plus sign (+) means it corrects farsighted astigmatism.

AXIS: This measurement is used to correctly orientate the cylinder correction when making your glasses. It is a number between 0 and 180 degrees and follows the CYL power if an eyeglass prescription includes cylinder power.
Its important to note that the right eye is always indicated as "OD" (oculus dexter) and the left eye as "OS" (oculus sinister) .
